According to numerous researchers, less than 25 percent of Americans have exercised their right to complete an advance directive. This leaves a vast majority of the population at risk of not having the opportunity to state their wishes regarding health care should they become incapacitated, either physically or mentally. A number of articles that involve advance directive completion are currently available. However, despite the growing number of nurse practitioners, the research is sorely lacking on the involvement of nurse practitioners on this topic. Research is also lacking on advance directive completion in the primary care setting despite the growing emphasis on preventive care. This project serves to support the hypothesis that by providing a multimodal means of education on advance directives in the primary care setting, the nurse practitioner will stimulate health care conversations, leading to an increased advance directive completion percentage.
